Introduction

This Client Agreement (“Agreement”) is entered into by and between Keys to Haven Therapeutic Services PLLC (“Therapist,” “we,” “our,” “us”) and the client (“you,” “your”) for the provision of therapy services. By scheduling and attending therapy sessions with us, you agree to the terms and conditions outlined in this Agreement. Please read this Agreement carefully.

Services

  1. Therapy Sessions: We provide individual, couple, and group therapy sessions, which may include in-person, teletherapy, or other forms of remote communication.
  2. Confidentiality: All therapy sessions are confidential. We will not disclose any information you share during sessions without your written consent, except as required by law (e.g., risk of harm to self or others, child or elder abuse).

Appointment Scheduling and Cancellation

  1. Scheduling: Appointments can be scheduled through our website [www.keystohaventherapeuticservices.com] or by contacting us directly at [contact@keystohaventherapeuticservices.com] or (210) 998-6827.
  2. Cancellation Policy: If you need to cancel or reschedule an appointment, please provide at least 24 hours’ notice. Cancellations made with less than 24 hours’ notice may be subject to a cancellation fee equivalent to the session fee.

Fees and Payment

  1. Session Fees: The fees for our therapy sessions are as follows:
    • Individual Therapy: $200 per session
    • Couple Therapy: $200 per session
    • Group Therapy: Depends on size of group. Average is $50 per person, per session
  2. Payment: Payment is due at the time of the session unless otherwise agreed upon. We accept payment via client portal submission using credit/debit card.
  3. Insurance: We do not accept insurance. At your request, we can provide you with a superbill you can submit to your insurance.

Client Responsibilities

  1. Participation: You agree to actively participate in therapy sessions and follow through with any agreed-upon treatment plans or recommendations.
  2. Communication: You agree to communicate openly and honestly with your therapist, including discussing any concerns or issues related to your therapy.

Therapist Responsibilities

  1. Professional Conduct: We are committed to providing services in accordance with professional and ethical standards.
  2. Confidentiality: We will maintain the confidentiality of all client information, except as required by law or with your written consent.

Termination of Services

  1. Client-Initiated Termination: You may terminate therapy services at any time. Please notify us if you wish to discontinue therapy.
  2. Therapist-Initiated Termination: We reserve the right to terminate therapy services if we determine that therapy is no longer beneficial or if you fail to comply with the terms of this Agreement. We will provide appropriate referrals if termination is necessary.

Limitation of Liability

To the fullest extent permitted by law, Keys to Haven Therapeutic Services PLLC shall not be liable for any direct, indirect, incidental, consequential, or punitive damages arising out of or related to your use of our therapy services.

Dispute Resolution

Any disputes arising out of or relating to this Agreement shall be resolved through confidential mediation before a mutually agreed-upon mediator. If mediation fails, the dispute shall be resolved through binding arbitration in accordance with the rules of [Arbitration Organization].

Governing Law

This Agreement shall be governed by and construed in accordance with the laws of FL, HI, TX / USA, without regard to its conflict of law principles.

Acknowledgment

By scheduling and attending therapy sessions with us, you acknowledge that you have read, understood, and agree to the terms and conditions of this Client Agreement.
